IMPORTANT HEALTH INFORMATION FOR SWMHS STUDENTS
. Notify School Nurse if your son/daughter has a health concern at the beginning of the school year, or when first diagnosed.
. NJ State law requires that any student with asthma must have an Asthma Action Plan completed by their physician and signed by a parent/guardian. A new Asthma Action Plan must be submitted for each new school year. Forms are available in the Nurses’ Office. Please be sure that the physician checks the spot allowing self-administration if indicated.
. Any student taking medication in school must have a medication form completed by a physician and signed by a parent/guardian. This includes all prescription and all over-the counter medications. Medication forms are available in the Nurses’ Office. New forms must be submitted each school year, and the medication, in the original container, should be brought to school by a parent.
. All students must have a current emergency card, so that parents/responsible designee can be contacted in case of illness or injury. Please notify the school immediately if there is any change in contact numbers.
. Any student with an allergy requiring the administration of epinephrine must have an Allergy Action Plan completed by their physician and signed by a parent. Forms are available in the Nurses’ Office. New forms must be submitted annually.
